Please read the description of each appointment type before booking.

• If you are suffering from a spinal disc issue, chronic low back or neck pain, or have been told that you are a candidate for spinal surgery, please select the “Trial/exam Spinal Decompression and Shockwave” appointment.
• Please only select the Focused Shockwave Therapy session if you have an active shockwave package.
• If you are currently in a care plan with Dr. Jacob, please select the correct treatment time as discussed with Dr. Jacob. A safe assumption is that you should book the same length treatment session as your last appointment.
• If you have never been seen by Dr. Jacob, you are required to book an examination before you can be treated.
• If it has been greater has been greater than 1 year since you have been seen, or you have a new injury that you have never sought treatment for, you are required to book a re-evaluation appointment.
